As part of our vision to ensure that every child, young person and adult in Ceredigion will be able to reach their full potential we are adopting an integrated service delivery model that will combine a range of current services and teams into early intervention, prevention and through age specialist services. This model will cover 4 main service areas:

  1. Initial Contact services – which will be the single access point into Porth Ceredigion Integrated Services for residents and external agencies 
  2. Porth Ceredigion – will be the decision making hub that will triage and determine the level of need for all cases and the best outcomes for those individuals. 
  3. Porth Cymorth Cynnar – will provide early help, prevention and intervention services with the aim of identifying need and offering support before issues and concerns become too great.
  4. Porth Cynnal – will provide specialist services for all clients based on need and not age, including support with Substance Misuse, Mental Health, Extended Support, Safeguarding and Planned Care.

This Corporate Manager role sits within Porth Ceredigion and will be the lead advisor within Porth Ceredigion on Targeted Intervention services. You will be responsible along with the Corporate Manager – Porth Ceredigion Targeted & Operational Support for the effective and efficient coordination and communication to both internal and external partners on all aspects of the Porth Ceredigion delivery approach.

The post holder will directly line manage all staff within the Profession Support team within Porth Ceredigion located at various locations within the County. 

Key responsibilities will include: 

  • To assist Corporate Lead Officers in the development of innovative proposals and business plans for the Targeted & Operational Support Service
  • To contribute to the setting, monitoring and control of team and service budgets 
  • To provide professional advice and support to internal and external stakeholders identifying options for action
  • To work with and support colleagues to provide advice and support in specific areas of expertise, formulating and implementing effective policies and individual support plans 
  • To work in close collaboration with other organisations and key partners to achieve maximum benefits for the Council and citizens of Ceredigion. 

As the successful applicant, you will: 

  • Apply strong management and leadership skills to ensure the effective and efficient management and delivery of the Porth Ceredigion integrated service.
  • Develop and improve Porth Ceredigion through adopting a responsive approach to identify ‘unmet needs’ with a view to maximising resources and outcomes for citizens;
  • Provide early support, prevention and early intervention to individuals at a disadvantage, including those who may be at risk and those who encounter challenges in their lives; 
  • To lead, motivate and encourage services and teams across numerous sites; 
  • Work collaboratively with corporate colleagues to develop and meet the vision for the integrated services model.
  • Have the ability to develop strong and trusted relationships, both internally and externally, with other agencies and partners.

Adult Services

We provide or arrange services for older people, people with disabilities or people who have problems with alcohol or drugs. We assist people with care and support in their own homes and with day services. We also provide residential care to those needing extra support.    Our primary functions are as follows: 

  • Housing: Affordable housing; Homelessness; Housing register; Welfare reform.
  • Direct Services: Day centres; Residential care homes; Resource centre.
  • Older People, Learning and Physical Difficulties: Autism spectrum disorder team; Enablement service; Porth Gofal adult triage.
