About the role

We are currently looking to recruit a Social Work Assistant to join the Integrated Triage and Assessment Team within Porth Gofal service area. This is an exciting opportunity to join the service with the implementation of the Through Age and Wellbeing transformation.

The Integrated Triage and Assessment team is at the heart of service provision within Ceredigion. It provides a decision making hub for all referrals that triages and determines the level of need and best outcomes for individuals. The service focuses on early intervention and recovery, supporting people to live as safely and independent as possible.

The Integrated Triage and Assessment team provides:

  • A responsive service to Citizens living in Ceredigion.
  • Applies a Strength based needs assessment focusing on service user choice and control using the Signs of Safety framework.
  • A safe service that identifies vulnerable and at risk Citizens, that responds effectively.
  • Provides information, advice and assistance about community health and social care services for individuals that improves the connection between people and their communities.
  • Encourages a balanced approach to the assessment of need ensuring that citizens are at the centre of conversations, decisions and the “what matters” conversation takes place.
  • Multidisciplinary triaging of referrals to identify the right professional to respond e.g. physiotherapist, Occupational Therapist or Social care workers to ensure right response at the right time.
  • Provide more consistent, coordinated assessment for care and support.
  • Improve the flow of information between services, resulting in faster decisions being made about people’s needs and support.
  • Reduces hospital admissions. Provides an in-reach service to reduce delayed transfers of care by providing timely discharges from hospital.

On a day to day basis you will:

  • Undertake assessments with individuals and their families.
  • Develop targeted goals and recovery plans.
  • Develop care and support plans for eligible care needs.
  • Work in partnership with professionals across all service areas.
  • Provide advice, information and assistance.
  • Use a whole family or network approach and signs of safety methodology.
