We are currently looking to recruit a permanent, Senior Practitioner Social Worker, to join the Integrated Triage and Assessment team, Porth Gofal Service.
This post is an exciting opportunity for a dynamic and committed member of staff to work in a multi-disciplinary environment which will support vulnerable individuals and their families within a hospital and community settings.
Porth Gofal is the integrated decision making hub for Ceredigion, that triages and determines the level of need of all cases and the best outcomes for those individuals.

Must have two years post qualifying experience with experience of supervising staff.
