About the role

We are looking to recruit an Individual and Family Support Coordinator for the mid of the county. This is to cover a period of maternity leave. Based within the Parenting and Family Support team. This is an exciting role within the service providing the opportunity to work with families from the community to create a bespoke plan to meet their family’s needs.

On a day-to-day basis you will:

  • create a multi-agency plan with the family
  • coordinate a whole family approach and develop a multi-agency meeting to discuss the support
  • liaise with the other agencies involved with the early intervention
