About the role

We are currently looking for a Team Manager to lead on the development of a new service for Ceredigion that will focus on providing specialised Disability Services. The service will focus on supporting individuals with a care and support assessment and have complex disabilities including Learning Disability.

The service is part of the Ceredigion Through Age Model and will work with all ages and in particular, transition age to adulthood and beyond.

The position will have a fundamental role in supporting a managing change process, moving from traditional services to the new model through a collaborative approach with colleagues from across the authority and partner agencies, e.g. Health.
