On a day-to-day basis, you will provide commissioning and brokerage advice, in relation to internally and externally provided social care services, on behalf of Ceredigion County Council, liaising with internal and external stakeholders, including providers, Health Board and care management teams.

You will also provide an effective and efficient brokerage service and arrange the delivery of external provider services, in a cost effective manner, that meets the specified requirements and assessed needs.
