About the role

Please note that the advertised salary for this position is subject to the pending 2026/2027 national pay award. The final salary will be adjusted accordingly once this has been agreed.

Newly Qualified Social Workers will commence on Grade 9 pay and job description before progressing to grade 10 after 2 years in post. Experienced social workers will commence at Grade 10 pay and job description. Both job descriptions can be viewed below. 

This position qualifies for a market supplement payment of £200 per month. Please note this additional payment will be reviewed on 31 March 2027.

Porth Gofal is the front door to Adult Social Care, bringing together professionals from across health, social care and the third sector to ensure people receive the right support, at the right time.

About the Role

As part of the Integrated Triage and Assessment Team, you will play a vital role in responding to contacts and referrals, carrying out timely assessments, and supporting adults and carers to build on their strengths. You will work with people at times of change or uncertainty, helping to prevent crises, reduce unnecessary escalation, and support positive outcomes.

You will:

  • Undertake strengths‑based assessments with adults and carers, focusing on what matters to them.
  • Develop and implement care and support plans that promote independence, wellbeing and recovery.
  • Work closely with health colleagues, housing services, community resources and the voluntary sector to deliver joined‑up, person‑centred support.
  • Champion early intervention and preventative approaches, supporting people to maintain control and live the lives they choose.
  • Use your professional judgement within a supportive team culture that values reflection and shared learning.

What We’re Looking For

  • A qualified and registered Social Worker (Social Care Wales registration required).
  • Experience of, or strong interest in, working with adults and carers.
  • The ability to make sound decisions in a fast‑moving environment.
  • Excellent communication skills and a commitment to collaborative working.
  • A values‑driven approach grounded in choice, dignity, independence and strengths‑based practice.
