About the role

Please note that the advertised salary for this position is subject to the pending 2026/2027 national pay award. The final salary will be adjusted accordingly once this has been agreed.

Newly Qualified Social Workers will commence on Grade 9 pay and job description before progressing to grade 10 after 2 years in post. Experienced social workers will commence at Grade 10 pay and job description. Both job descriptions can be viewed below. 

This position qualifies for a market supplement payment of £200 per month. Please note this additional payment will be reviewed on 31 March 2027.

An exciting opportunity has arisen within Ceredigion’s Integrated Triage and Assessment Team to join a newly developed hospital-based social work service. We are looking for a committed and compassionate Social Worker to work in our acute site at Glangwili Hospital, Carmarthen supporting timely and effective discharge planning and strengthening the interface between health and social care. 

Key Responsibilities: 

  • Work closely with health professionals tofacilitatepre-referral discussions, reducing delays and preventing unnecessary referrals to social care. 
  • Undertake proportionate assessments and contribute to Discharge to Assess (D2RA) pathways in collaboration with Occupational Therapy colleagues.
  • Provide information, advice, and guidance at the point of contact within the hospital setting.
  • Support the front-door turnaround approach, enabling access to care and support plans and follow-up assessments in the community.
  • Contribute to risk assessments for the Targeted Care & Enablement Service.
  • Build effective working relationships with hospital staff to support decision-making around care pathways.
  • Work under the guidance of a Senior Practitioner, who will lead on complex cases and coordination.

What We’re Looking For: 

  • Qualified Social Worker registered with Social Care Wales.
  • Experience in hospital discharge planning, assessment, and multi-agency working.
  • Strong communicationand interpersonal skills.
  • Knowledge of relevant legislation including the Social Services and Well-being (Wales) Act 2014.
  • Ability to work collaboratively in a fast-paced environment.
