Our Through Age and Well-being Strategy sets out how we will develop a skilled and innovative workforce who will provide through age services that focus on preventative support and early intervention with easy access to information, advice and assistance, proportionate assessment of need and care and support plans for those who need longer term help.

The Early Intervention service provides preventative through-age person centred support. There are two teams within the service as follows:
Carers and Community Support Team
The Carers and Community support team develop opportunities, and services that support people in their communities. The team works with people of all ages connecting them to individual and group support accessible to them. We work in close partnership with colleagues across the Local Authority, Health and with those in the 3rd Sector to ensure people get the right support at the right time. As well as the team being through age, and working with the general public, within the team there are also a focus on specialist areas, namely Ageing Well in Ceredigion, the Family Information Service, the Community Connectors and the Carers Information Service.

Parenting and Family Support Team
The Parenting and Family Support Team provide a breadth of support for families, children, parents and carers. The team focusses on prevention and early intervention to promote successful outcomes for those with whom they work. The responsibility for co-ordination of Teams around the Family (TAF) sits within the team, with holistic multi agency packages of support being provided. Family support is provided by our Family Support Workers who are able to work with families/ carers of children aged 0-18. The Flying Start programme is also organised within this team and is a partnership programme with Hywel Dda Health board providing targeted support to children aged 0-3. Support is delivered on an individual and group basis and there are a myriad of parenting programmes available along with bespoke support. The team manage several Family Centres and Integrated Children’s Centres in the county. The team is also able to provide direct support to children and young people aged 0-25.
