Our Through Age and Well-being Strategy sets out how we will develop a skilled and innovative workforce who will provide through age services that focus on preventative support and early intervention with easy access to information, advice and assistance, proportionate assessment of need and care and support plans for those who need longer term help.

Extended Support Team Overview

This service is currently made up of 3 staff groups:

The Children with Disabilities Team:

  • Team Manager
  • 1 x Social Worker Senior Practitioner
  • 1 x Occupational Therapist
  • 3 x Social Worker – generic disability
  • 1 x Social Worker – specialist in Autism
  • 2 x project worker

The Community Team for Learning Disabilities:

  • Same team manager as children with disabilities team
  • 1 x Social Worker Senior Practitioner
  • 1 x Occupational Therapist
  • 3 x Social Worker – generic disability
  • 1 x Social Worker – specialist in Autism
  • 2 x project worker

All Team Manager, Senior Practitioners (except for the occupational therapist role) and social workers will have a social work qualification. As part of their role they will have a good understanding of the relevant legislation and apply this in their daily practice. They will also have a good knowledge base of disabilities and how this impacts on the service user’s day to day life. The Specialist in Autism social workers will have good knowledge of the Autism Code of Practice and be able to embed this with their colleagues in the team. The Social Workers will also be willing to work towards undertaking Best Interest Assessors training to be able to carry out DoLS assessments as and when needed.

The Social Work Assistants and Project Workers will need a basic understanding of the Social Services and Well-Being (Wales) Act 2014 and experience of working with children and adults with disabilities.

The Mental Capacity Act Team:

  • Team Manager
  • 3 x Senior Social Workers

The team manager and senior social workers will have a social work qualification. They will be qualified to carry out best interest assessments and undertake Deprivation of Liberty assessments. They will have a good understanding of the Mental Capacity Act and will be engaging in all opportunities to develop a good level of undertraining of the Liberty Protection safeguards.
