Planned Care Team Overview

Children’s Team

The Children’s Team works closely with partner agencies to support children and young people and their families who have care and support and safeguarding plans. This includes child in need of care and support, child protection, looked after/care experienced, care leavers and those with permanency plans to remain with family members or be adopted.

The Senior Practitioner social worker role is an experienced social worker professional working with a small complex case load and with the duties of a social work supervisor.

Social Workers will be expected to undertake the statutory duties under the Social Services and Well-being (Wales) Act 2014 and other key legislation to achieve good outcomes for children and families including court work when required.

Adoption Social Workers will be specialist Social Workers undertaking statutory duties under the Adoption and Children Act 2002 and the Social Services and Well-being (Wales) Act 2014 as part of a Regional Adoption Service.

The The Personal Advisor role is to provide support for eligible Care Leavers and carry out duties in relation to The Care Leavers (Wales) Regulations 2015role is to provide support for eligible Care Leavers and carry out duties in relation to The Care Leavers (Wales) Regulations 2015

The NEET Worker role is to provide support for looked after children and care leavers who are not in employment education or training in order to access positive opportunities.

Social Work Assistants will carry out support functions in conjunction with the Social Workers in the team and they will be able to seek opportunities to seek social work qualification if they wish.

The Education Worker will work closely with schools to support the education needs and statutory plans of looked after children.

The Activity Co-ordinator will help with organised targeted activities for looked after children and others to support their well-being and placement stability.

The Senior Family Contact Worker ‘s role is to co-ordinate and deliver supervised or supported contact arrangements for children in care.

Family Support Workers will carry out supervised or supported family contact sessions and other direct support roles with children and young people and families.

Adult’s Team

The Planned Care Service – Adult Team’s primary role is working with Older Adults and their carers, supporting them to achieve their wellbeing outcomes, as in line with the statutory duties under Part 3 and 4 of the Social Services and Well-being (Wales) Act 2014.

The role involves comprehensive assessments of need, carers assessments, care management and review of care plans, provision of services to those assessed as requiring a care and support plan by assisting individuals to resolve major difficulties, enhancing their coping skills & establishing their resilience and independence.

Staff will be required to work within current Legislation & Statutory Guidelines as well as county Policy, procedures & practice in order to assess, commission & review support services and planned interventions for those who are at risk or vulnerable

In addition staff are required to ensure quality of services with colleagues in the statutory & independent sector, to ensure multi-agency approach in empowering service-users to speak for themselves whilst ensuring they are safe & protected.

Suitably qualified staff will work within  statutory and legal requirements, and within frameworks set out in the policies and procedures of the agency as well as SSWBA, Mental Capacity Act, part of the role you will be required to undertake Best Interest decisions and also support adults who lack mental capacity with all aspects of their personal property and financial affairs regulated under the Mental Capacity Act.
